Assistive Technology Loan Application: Part 4

Information You Must Provide

You must verify the following information (if relevant) in your loan application packet:

1. Income (for one month)

  • Earnings � Pay slip or check stubs
  • Social Security Statement
  • SSI Statement
  • State Supplement
  • ADC Statement
  • Food Stamps Verification
  • Fuel Assistance Documentation
  • Rental Income (verification or letter from boarder/tenant)
  • Child Support/Alimony
  • Interest & Dividends
  • Documentation of other income

2.Expenses (for three months)

  • Rent/mortgage � rent receipts, letter from landlord confirming payment regularity, or evidence of mortgage payment schedule
  • Utility statements or canceled checks or money order receipts for 3 months (phone, heat, electricity, gas)
  • Property Taxes
  • Property Insurance
  • Vehicle (car, truck or van) � loan & insurance
  • Ongoing Account: credit card payment, etc.
  • Payment of Child Support/Alimony

3. Bank Records

  • Accounts
  • Securities
  • Loans

4. Written Statement from Vendor

  • Equipment or device
  • Needed alterations

5. Special Requirements

  • If your current rental has been less than one year, you may need a letter from your previous landlord to verify your payment record.
  • If the loan is for home modifications and you live in rental property, you will need a letter from your landlord permitting alterations to the property and confirming your financial reliability. The letter should also state that you will be permitted to remain in the residence for the length of time for which the loan is written. Remember, if you move out, your landlord may require you to return the property to its original design.
  • If the loan is for modifications to a residence that you own, you should provide a copy of the property title or tax bill.
  • If the loan if for a vehicle or vehicle modifications, you must provide proof that you have collision insurance on the vehicle. If you own the vehicle or you are buying a new vehicle, it will be used for collateral.
  • If your loan is to be co-signed, you will need to supply all of the listed documentation and verification for all who will co-sign for you.
  • If you are applying in the name of an individual for whom you are Conservator of Estate or for whom you have Power of Attorney, you will need to submit written verification of your legal status.

6. Complete the Loan Application

To complete the loan application packet, you will also need to submit your financial budget work sheet and you Individual Assistive Technical Plan. You should copy the application packet and all your documents for your records before you send them to the bank.
